What is FACT?
Florida Assertive Community Treatment (FACT) is a community-based service offering services to adults who have been diagnosed with a serious mental illness. FACT’s goal is to give community care and help those have a life that isn’t dominated by mental illnesses.
We are a team of community-based professionals (psychiatrists, nurses, mental health counselors, substance abuse specialists, care managers and employment specialists) that provide individualized ongoing care.
What FACT does?
The FACT team can assist with medical and mental health issues, as well methods to help make the best of each day. We also help clients find housing, apply for food stamps, go back to school, get a job, and so much more. We collaborate closely with clients to achieve their utmost priorities and goals.
We meet clients where they are to offer services, in home, at work or in other settings where they need support.
Our team is dedicated to the clients success in life and in the community. This is a longer term approach to assist clients with gaining independence. Clients will always be able to contact someone on our team.
FACT can help client if they have any of the following:
· Numerous visits or admissions to the hospital
· Repeated jail time and/or homelessness
· Difficulties making or keeping friends
· Thoughts of hurting themselves or others
· Feeling like people can read their mind
· Thinking people are out to get them
· Hearing and/or seeing things other people can’t
· Difficulty organizing thoughts
· Current or past issues with substance use/abuse
We can help clients stay in the community, find stable housing and employment. We help clients live independently, build a stronger support network, and successfully manage health and mental wellness.
