Adult Case Management

Case Management provides a single point of coordination to help individuals with severe and persistent mental illness identify services and/or supports they need; assist them in receiving person specific services and/or supports in order to maximize independence, quality of life and recovery. Individuals receive assistance utilizing personal strengths, preferences, and abilities. The person's cultural, religious, and educational background contributes in the recovery process.

Individuals, in partnership with the case manager, assess strengths & needs and develop a service plan as a roadmap for the path to recovery. Others involved with the persons' recovery are invited to join in this process. A comprehensive orientation is provided so that all involved are aware of the roles each will play. Case Managers are able to provide assessment, advocacy, linkage, monitoring, and planning. All respects to privacy and confidentiality are given in accordance with Federal and State law. Length of case management services is determined by the individual requesting assistance.

Referral sources include Meridian Access Center, community referrals, crisis stabilization and detoxification units, county health departments, outpatient programs and service assessment teams.
