Job Summary

This is a highly advanced professional counseling position providing addictions assessment, evaluation intake, treatment planning and documentation addressing the specific treatment needs of clients, as well as consultation services to Center and other agency staff. This position requires the ability to build professional, empathic and caring and supportive relationships with staff, while providing efficient and appropriate direct clinical services.

Duties and Performance Standards
  • 1. Perform clinical assessments to determine diagnosis, level of care, treatment plan issues and/or make recommendations to other agencies about these issues. Evaluations include identification of substance abuse and mental health problems and other problems that have resulted from or contributed to the cycle of addiction and/or mental illness. Clinical impressions are clear and give a detailed picture of how the client's bio-psychosocial spheres have been impacted and identify specific strengths, needs, abilities/interests, preferences and challenges/barriers and how these factors impact treatment participation and likely outcome. Standard: ASAM, FARS/CFARS and Peer Review reflects initial assessments leading to appropriate diagnosis, level of care and initial treatment plan.
  • 2. Assists clients in the development and implementation of individual treatment plans addressing substance abuse and/or mental health problems and other problems that have resulted from or contributed to the cycle of addiction and mental illness. Treatment plans are based on a recovery and resiliency model, are developed with the client and are written in language appropriate to the client. Objectives are deemed achievable within the treatment plan time frame and are age/development level appropriate, interventions reflect evidence based and best practices standards. Standard: Peer Review reflects appropriate diagnosis, level of care and treatment planning.
  • 3. Completes comprehensive reassessment prior to expiration of current treatment plan. The reassessment gives an accurate portrait of the client's progress toward achieving goals and objectives. The assessment provides justification for continued services and changes in the treatment strategy to assist the client in achieving the goals on the new treatment plan. A review of the diagnosis is done and recorded. The reassessment reflects the appropriate level of care for the client and a revised treatment plan is completed prior to new services being provided. The reassessment also includes updated FARS/CFARS, ASAM, MH PIDS, consents and releases as indicated by policy and procedures.
  • 4. Discharge/transition planning reflects criteria to be met prior to discharge/transitioning from services including improvement in FARS scores. The plan includes specific services the client will need to make a successful transition and linkages to these services are in place prior to planned discharge date.
  • 5. Provides Outpatient/Intervention/Rehab services to clients needing that level of treatment in order to promote the restoration of skills needed for independent living and life management that have been adversely affected by the client's substance use and/or mental illness. Services combine education, skills development and practice, social support and network enhancement, and coordination of other services as needed for each client. Treatment utilizes both group and individual counseling.
  • 6. Provide clinical services in schools, client homes and in the clinic. Provides crisis intervention services for clients as needed.
  • 7. Provide services to assigned clients and families that include information about symptom management, medication and treatment options. Provides family counseling when clinically appropriate. Standard: Peer review reflects appropriate services to client and family. Standard: Provides services to clients who meet criteria for the appropriate level of care.
  • 8. Acts as Center liaison in the coordination of client services with appropriate community agencies; provides consultation to these agencies as needed to serve as advocate for the clients and the Center while working to insure the provision of needed services for the client (includes off-site case conferences and court appearances). Standard: Provides consultation at agencies as requested or as dictated by client needs. As evidenced by Peer Reviews reflecting appropriate follow-up. Standard: Review of clinical records indicates appropriate feedback has been provided in a timely manner to referral sources and/or courts.
  • 9. Facilitates continuity of care within the agency and community for assigned clients. Standard: Peer Review reflects appropriate liaison and referral.
  • 10. Documents all services provided in a timely and accurate manner consistent with Center policy and procedures, best practice standards reflected in Peer Review scores averaging > 1 and Documentation Audit scores > 98% for Medicaid, and > 95% for others.
  • 11. Assist in the orientation/training/supervision of students, interns and new clinicians within area of knowledge and under the guidance of the program director. Standard: Provides general orientation to students and new Clinicians Standard: Provides training to new members within area of knowledge/skills
  • 12. Provides sufficient services to generate projected revenue (prorated for portion of year during which employee was with Meridian). Obtains and maintains insurance authorizations, verifies fee schedules are up to date and accurately reflect the client's ability to pay, and ensures clients are current on all fees. Accurately records all services provided to clients and hands in billing notes no later than 5 pm on Monday for the prior week.
  • 13. Complies with needed administrative documentation for payroll, travel reimbursement, etc.
  • 14. Participates in the Centers QI process as asked or required. May initiate activities within his/her own area of responsibility to improve efficiency, effectiveness or quality. Standard: Provides requested data and problem identification, participates on teams and program meetings where quality issues are addressed. Standard: Returns completed client satisfaction surveys in a timely manner.
  • 15. Assists in the identification of solutions to identified programming problems and performs tasks related to program improvements as may be identified. Standard: Participates in team program development efforts.
  • 16. Ensures compliance with ADM Performance Standards
  • 17. Aligns individual goals, objectives and daily activities with program, departmental and organizational goals and objectives
  • 18. Maintains current First Aid, CPR, TEAM and other professional training as required by contract with the State and other accrediting bodies. Pursues additional training in Addictions and Counseling to insure continued professional growth while maintaining knowledge of current trends in addictions and mental health. Standard: Reflected in Human Resources training reports, workshop completion certificates and continuing education credits.
  • 19. Communication Skills Standard: Communicates effectively orally and in writing with internal and external customers

  • 27. Flexibility Standard: Willing to take on new tasks and responsibilities as needed.
  • 28. Performs other duties as required to meet program and departmental goals and objectives Standard: Responds promptly and accepts assignments readily.
Minimum Qualifications

Masters degree in social work, counseling, psychology or related field, or a Certified Addictions Professional. Prefer three years of experience, with at least 2 years working with children.
