Children’s Case Manager in Gainesville

Job Summary
This is an independent professional position performing comprehensive assessments, service brokerage, and advocacy services for severely emotionally disturbed children and their families. The Case Manager works with other professionals from numerous community organizations as well as with Center staff. The work is performed with a high degree of independence; however, work is reviewed through conferences, supervision and required reports. This position involves managing a caseload of severely emotionally disturbed children
General Duties
- 1. Assists Program Director in supervising and training other case managers.
- 2. Completes a comprehensive assessment to identify service needs.
- 3. Develops a service plan with child and family’s input, identifying specific goals and objectives for meeting service needs.
- 4. Links the client and family with services and resources identified in the Service Plan.
- 5. Maintains contact with all staff/agencies providing services for the purpose of monitoring clients progress and assessing appropriateness of services.
- 6. Serves as an advocate for the client with a wide variety of community agencies and Center programs. Specifically, coordinates with the Department of Children and Families (DCF), the local judicial system, schools, the Department of Juvenile Justice (DDJ), Family Service Planning Team (FSPT) and Case Review Committee (CRC).
- 7. Conducts face-to-face visits to monitor progress in meeting the Service Plan goals and objectives.
- 8. Maintains a caseload of approximately 20 children and their families.
- 9. Prepares and maintains client records and insures quality of documentation of medical records.
- 10. Records every activity on behalf of a client on a daily basis via Progress Notes.
- 11. Responsible for meeting yearly productivity and revenue goals.
- 12. Researches, develops and maintains a comprehensive list of community resources that includes addresses, phone numbers, and names of contacts for each resource.
- 13. Performs other duties as required to meet program and departmental goals and objectives.
Minimum Qualifications
BA/BS from accredited university with major course work in the areas of psychology, social work, health education or a related human services field and must have a minimum of one year of experience working with children and/or adolescents who have serious emotional disturbance.
Licenses
Must possess a valid drivers license and have acceptable driving record as determined by the Risk Management Department. Must possess viable transportation maintained according to manufacturer's specifications, which is covered by personal auto insurance.
