Adult Case Manager in Lake City

Salary : $30,000.00

Job Summary

This is an independent professional position performing comprehensive assessments, service brokerage, and advocacy services for adults with severe and persistent mental illness (SPMI). The Case Manager works with other professionals from numerous community organizations, as well as with Center staff. The work is performed with a high degree of independence; however, work reviewed through conferences, supervision and required reports. This position involves managing a caseload of persistently mentally ill adults.

General Duties
  • 1. Completes comprehensive assessments, assessing the total needs of the client.
  • 2. Develops, with the client’s input and agreement, a Service Plan with specific goals and objectives to resolve service needs.
  • 3. Links the client and family with services and resources identified in the Service Plan.
  • 4. Maintains contact with all staff/agencies providing services for the purpose of monitoring clients’ progress and assessing appropriateness of services.
  • 5. Serves as an advocate for the client with a wide variety of community agencies and Center programs. Promotes community program development to meet client needs.
  • 6. Conducts face-to-face visits to monitor progress in implementation of the Service Plan.
  • 7. Prepares and maintains client records and insures quality of documentation of medical records.
  • 8. Records every activity engaged on behalf of a client on a daily basis via Progress Notes.
  • 9. Responsible for meeting yearly productivity and revenue goals.
  • 10. Researches, develops and maintains a comprehensive list of community resources that includes addresses, phone numbers, and names of contacts for each resource.
  • 11. Helps to enlighten and broaden public awareness about mental illness.
  • 12. Monitors clients residing in a State Facility, i.e. NFETC, NEFSH, FSH and maintain monthly contact with the facility treatment team in order to develop timely discharge plans.
  • 13. Performs other duties as required to meet program and departmental goals and objectives.
Minimum Qualifications

Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, sociology, criminal justice, rehabilitation, or a related human services field and 1 year experience working with adults experiencing serious mental illness

Bachelor’s degree from an accredited university or college in any field with 3 years experience working with adults experiencing serious mental illness.

Must complete ADM approved mental health targeted case management training within six months of employment..{MH Targeted Case Management Coverage and Limitations Handbook, April 2002}

Must be able to type 24 WPM and pass a proficiency level in Outlook and Windows.

Must possess a valid driver’s license and have acceptable driving record as determined by the Risk Management Department. Must possess viable transportation maintained according to manufacturer's specifications, which is covered by personal auto insurance.
